Controlled Trial
A controlled trial tests an intervention by comparing outcomes in otherwise comparable groups that differ in whether they receive it. The gap between their outcomes is the causal signal.
One group of sailors received lemons and escaped scurvy. Another went without lemons and developed it. The persuasive fact was not merely that lemons accompanied recovery; it was that withholding them produced a different outcome.
E1Create a counterfactual you can observe
Causal questions contain a hidden comparison: what would have happened to the same people without the intervention? You cannot observe both versions of one person at once, so a controlled trial builds an approximation. A treatment group receives the suspected cause; a comparable control group does not. If their outcomes diverge, the intervention becomes the clearest candidate explanation. This is the step that turns an association into a causal test, sharpening Correlation vs Causation.

Lemons against scurvy
James Lind's sailors supplied both sides of the comparison: scurvy was absent among those given lemons and present among those denied them. The untreated group revealed the outcome that the treated group might otherwise have suffered.

Different outcomes do not isolate the intervention if the groups also differ in other important ways. A controlled trial is strongest when treatment assignment is the clearest systematic difference—the additional protection formalized by a Randomized Controlled Trial.
Run two versions, not one
Before adopting a change tomorrow, choose one measurable outcome and create a control: apply the change to one comparable case while leaving another unchanged. Decide the measurement and time window in advance, then judge the intervention by the difference between the two outcomes.
