A randomized controlled trial isolates causation by creating comparable groups and making the suspected intervention their clearest systematic difference. If outcomes do not diverge, the intervention is unlikely to be the cause.

Hans Selye injected rats with a hormone and watched them develop alarming problems. Then the rats injected with harmless saline developed the same problems. The apparent culprit was not the hormone at all: both groups had endured the same stressful handling.

Make the treatment the only patterned difference

An outcome after an intervention proves little by itself. Recovery might reflect expectation, pre-existing differences between participants, an unnoticed confounder, or even what observers expect to find. An RCT adds a control group, then uses random assignment to spread known and hidden differences across the groups. With a sufficiently large sample, those differences tend to cancel out rather than cluster systematically.

Now the groups travel through the experiment together while differing mainly in treatment. The causal signal is therefore not what happens in the treated group, but the difference in outcomes between groups. This turns an association into a sharper question: what changed when the suspected cause—and little else—changed?

Randomization reduces bias; it does not abolish it

RCTs are designed to counter placebo effects, selection bias, confounders, and observer bias, but their credibility still depends on how the trial is designed and conducted. A comparison is only informative when the groups are genuinely comparable and the experiment preserves that comparison.

Ask for the missing group

Tomorrow, when someone claims that a treatment, policy, or habit produced an outcome, write down what happened to the closest comparable people who did not receive it. If there is no credible control group—or assignment let participants select themselves—treat the claim as a lead to investigate, not a causal conclusion.
